The purpose of this website is to provide stories which health professionals can tell their patients to help with their self management.

An assessment plan is provided for each story which allows the health professional to assess whether the patient understands key concepts or has the skills needed to perform a task.

A template for a patient (client) plan is also provided, which is based on health coaching where the patients prioritises a set of tasks they are willing to do and changes they want to make.

 Stories & key concepts
 

The stories are broken down into key concepts and grouped with a maximum of 6 concept, but preferably with less.

These concepts provide the knowldge and skills needed when a person with diabetes is asked whether they want to make behavioural changes or perform tasks which will help with their self management.

 Assessment
 

The health professional should be able to quickly assess whether the patient understand the key conscepts or is competent at persforming a task.

The recommended method is either Tell or Show.

The patient is asked a question which requires them to TELL something or SHOW how to perform a task

The health professional can now make a simple assessment by grading the clients ability by rating by rating their understanding or competence:

Y - Yes, the patient understands or is able to do what is required

? - Don't know, if the patient understands or is able to do what is required

N - No, the patient doesn't understands or isn't able to do what is required

A rateing of ? or N means that further education on these comptencies will be needed.

 Client education plans
 

The health education plan is created with the help of the patient.

The health professional writes down a maiximun of either:

     - 2 behavioral changes or tasks they would like the patient to do.

     - 4 tasks they need to know

     - or, 6 pieces of knowledge (concepts) they need to remember

The patient then rates the changes or tasks:

     Y - Yes, I really want to make the change

     ? - I would be a good idea but...

     N - No, not now

The health professional can use the results to set agreed patient goals.

These goals need to be reviewed with the patient, at a later date, when the client self rates their achievement:

     Y - Yes, I have reached my goal

     ? - I would have but ...

     N - No, I couldn't

Using an educational plan helps both the health professional see what has been taught. It also helps the health professional see what else needs to be covered.
